The key question
CDC identifies heart disease and stroke among diseases caused by smoking. Secondhand smoke also harms cardiovascular health in people who do not smoke. A cigarette is therefore not merely a lung exposure, and choosing a lighter-sounding product does not establish cardiovascular safety. Quitting reduces cardiovascular risk over time, including for people with existing coronary disease. Individual risk depends on more than appearance, fitness, or the absence of a cough. [1] [2] [3]
Separate nicotine from smoke exposure
Nicotine drives dependence, while tobacco smoke adds many toxic exposures. E-cigarettes have a different exposure profile, but aerosol is not simply harmless water. The relevant question is not only whether one product contains fewer substances than another: age, pregnancy, secondhand exposure, and continued cigarette use change the context. Avoiding initiation and supporting cessation are different conversations, and both should use clear information without blaming people who need help. [4]
