The key question
Nicotine helps sustain dependence, but cigarette smoke contains many other harmful substances. Explaining every smoking-related disease as nicotine alone misses the effects of combustion. CDC links smoking with cancer, cardiovascular disease, and lung disease. This distinction also matters when discussing professionally guided quitting treatment: a medicinal product and inhaling smoke are not identical exposures. It does not make nicotine harmless, especially for developing brains and during pregnancy. [1] [2]
Separate nicotine from smoke exposure
Nicotine drives dependence, while tobacco smoke adds many toxic exposures. E-cigarettes have a different exposure profile, but aerosol is not simply harmless water. The relevant question is not only whether one product contains fewer substances than another: age, pregnancy, secondhand exposure, and continued cigarette use change the context. Avoiding initiation and supporting cessation are different conversations, and both should use clear information without blaming people who need help. [2]
